The candidates selected and appointed as Junior Data Entry Operator in Odisha Secretariat vide Order No.22951 Dt.01.07.2021 are to furnish the following documents in Original together with self-attested Xerox copy of them at the time of Joining for verification by the Administrative Department.

- Medical Fitness Certificate from concerned CDMOs/PHOs
- Two Character Certificates
- No Criminal Antecedent / Case Certificate
- Oath of Allegiance to the Constitution of India
- Declaration of non-contact of plural marriage
- Caste Certificate (ST/SC/SEBC as applicable)
- Certificate for Persons with Disability (as applicable)
